Report Requests
Attorney Requests
The Summit Fire Department will honor requests as outlined below.
Requests sent via facsimile or requests that do not meet the guidelines set forth herein will be destroyed upon receipt and will not be honored.
Subpoena Requests
Upon receipt of a Subpoena, the Summit Fire Department will verify the validity of the Subpoena with the issuing Court. Once verified, the request will be processed per the requests outlined in the Subpoena. If you would like a faster return please include an email address in the initial correspondence and a copy of the compliance will be emailed to you at the time of processing along with a formal mailing under the terms of the Subpoena.
Authorized Requests
Requests that are authorized under the Health Information Privacy and Portability Act (HIPPA) (PDF) will be honored only if the following items are included with the request.
- A properly signed and executed Summit Fire Department HIPPA Privacy Authorization Form
- A check made payable to The Summit Fire Department in the amount of $25
- An official request on the attorney's letterhead outlining the date of the incident, the Patient's name and the Date of Birth
If you would like a faster return please include an email address in the initial correspondence and a copy of the compliance will be emailed to you at the time of processing along with a formal mailing.

Individual Requests
The Health Information Privacy and Portability Act (HIPPA) (PDF) sets specific guidelines regarding requests for medical information. Your ambulance report is considered "protected" information. Under HIPPA, an individual may request a copy of his or her own medical records. To obtain a copy of your ambulance report contact the EMS office of the Summit Fire Department at 708-594-3948 to schedule a time to pick up your records.
Requests over the phone can not be honored under law. You must be present, have valid identification and sign an official request for your records. If the patient is a minor, a parent or legal guardian may obtain a copy of his or her medical record. You will be required to show proof of your relationship to the patient.
